College Day Request Form: These yellow forms are available outside the high school guidance office. Submit the completed form to Mrs. Riddle at least two days prior to your absence.
BBS Information Release Form: These blue forms are available outside the high school guidance office. Fill this out to request a transcript, test scores, or other information to be sent to a college, scholarship organization, or yourself.
Seniors are asked to complete the Senior Reference Form and return it to the Guidance Office as soon as possible. The form provides information the guidance counselors use when writing recommendation letters for scholarships and other awards.

Athletes who are interested in competing on the college level should visit this site to complete the NCAA Clearinghouse form.
